WILL MY INSURANCE COVER THE TREATMENTS?
Dr. Wolf and Sophia Bouwens are in-network with United Healthcare, Optum and Bind Benefits. Dr Wolf also accepts referrals through the Veterans Community Care program. Health plans vary, and not all plans cover acupuncture. Prior to booking an appointment, please call our office and provide your name, date of birth, and insurance ID and group # so that we can verify your acupuncture benefits.
If you have a health insurance company not listed above, we can provide you with a superbill you can submit to your insurance company to see reimbursement. You will be required to pay up front for the services, as we do not bill insurance companies as out-of-network providers.
Dr. Wolf, Sophia Bouwens and Michael Koziol accept and bill for motor vehicle accident claims and workers compensation claims. A written referral from a medical doctor is required before starting care.
We do not accept any other types of insurance at this time.

Initial Consultation for complex neurological cases, concussions and TBI: 120-minute: $299
Follow-up 60 minute appointment: $120
Follow-up 45 minute appointment: $95
Follow-up 30 minute appointment: $75

WHAT CAN I EXPECT ON MY FIRST VISIT?
At your first visit we will take the time get to know you and the conditions you want help with. We will perform a functional neurological exam and based on our findings, create an individualized treatment plan to help you find the relief you are seeking.
HOW ARE YOU HANDLING COVID-19?
We follow the guidelines set forth by the state of Minnesota regarding masks. We have air purification systems in place that include UVC-light and HEPA filtration to ensure our air quality is excellent. We sanitize surfaces in between patients and maintain a low-traffic area.
