WILL MY INSURANCE COVER THE TREATMENTS?
Michael Koziol and Sophia Bouwens are in-network with United Healthcare, Optum and the Veterans Community Care Program. Within UHC, health plans vary, and not all plans cover acupuncture. Prior to booking an appointment, please call our office and provide your name, date of birth, and insurance ID and group # so that we can verify your acupuncture benefits.
If you have a health insurance company not listed above, we can provide you with a superbill you can submit to your insurance company to seek reimbursement. You will be required to pay up front for the services, as we do not bill insurance companies as out-of-network providers.
Our providers all accept and bill for motor vehicle accident claims and some workers compensation claims. A written referral from a medical doctor is required before starting care.
We do not accept any other types of insurance at this time.

Initial Consultation 60-minute: $149
Initial Consultation 90-minute: $199-249 depending upon provider
Initial Consultation 120-minute for complex neurological cases, concussions and TBI: $349
Follow-up 60 minute appointment: $129
Follow-up 45 minute appointment: $95
Follow-up 30 minute appointment: $75

WHAT CAN I EXPECT ON MY FIRST VISIT?
At your first visit we will take the time get to know you and the conditions you want help with. We will perform a functional neurological exam, orthopedic assessments and based on our findings, create an individualized treatment plan to help you find the relief you are seeking.
